Saturday 11/17/2012 Round Table Pizza LOP 10:00am–1:00pm
Saturday 12/08/2012 Round Table Pizza LOP 10:00am–1:00pm
Saturday 01/12/2013 Round Table Pizza LOP 10:00am–1:00pm
Thursday 01/17/2013 Round Table Pizza LOP 6:00pm–8:00pm

ITEMS YOU MUST BRING THIS YEAR TO REGISTRATION (RETURNING & NEW):

To save time PLEASE BRING COPIESof the below items
1.  A COPY of PLAYERS Certified birth certificate… (not a certified abstract):
to show proof of child’s age. Division assignment based on player age as of 4/30/2013.

2. COPIES OF 3 Documents for Proof of Residency

(Items MUST be dated between 10/1/12 -2/15/13):
BEST ITEMS ARE:
* COPY of your Drivers License
* COPY of your Auto Insurance (dated between 10/1/12 -2/15/13)
* COPY of your Auto Registration (dated between 10/1/12 -2/15/13)
If you don't have 1 of the above items substitute with:
* COPY of PG&E/NID/Phone Bill (dated between 10/1/12 -2/15/13)
* COPY of Cable Bill (dated between 10/1/12 -2/15/13)
* COPY of Insurance docs (dated between 10/1/12 -2/15/13)
to verify player’s residence is within league boundaries.

3. Personal Calendar…. for membership commitment (time commitments) planning

4. 2nd Check…. Will be requested it will not be cashed unless you do not fulfill your membership commitment. Amount is determined by the # of players you have.
